I am a few turns into a new map and just noticed that I can see a partial enemy flag on an uninvolved country and when I mouse over it it shows as a secret enemy ally. This is a bug, right? There is no espionage option I am unaware of, right?

This is not a bug.
Enemy has 2 medium-size countries and there are 2 medium-size countries => they are his allies.

I guess there is espionage too. Sometimes I get a box with a message where my spies learned secret enemies. :o

But this is very rare.

It's not really espionage - just that when mathematically it becomes apparent that a country is a secret ally the game is kind enough to tell you.

If it it your ally, you get a message telling you that the enemy knows.
if it is an enemy ally, you will see the unfurled flags in the cities.

If games are played on high density setting, then this is very common.
On lower densities, it's quite rare.
